The number one side effect of sublingual immunotherapy (SLIT) is significant allergy relief and reduced dependence on antihistamines and nasal sprays.

Sublingual immunotherapy is effective for 85% of patients. It requires placing a small amount of allergen extract under the tongue. It is a safe and effective alternative to allergy shots, with fewer side effects. However, like any medical treatment, SLIT can cause some side effects, which are generally mild and temporary. In general, sublingual immunotherapy is very safe. Some common side effects may include itching, burning, or irritation or some allergy symptoms after the drops are taken.  When these occur, they tend to occur right after the drops are taken within 10-30 minutes. These symptoms tend to pass quickly, and if they occur, often we recommend considering an antihistamine to lower them. With time, symptoms tend to improve.

Signs of a rare but potentially severe reaction include shortness of breath, tongue or lip swelling, cramps, vomiting, and a sense of doom (please refer to your allergy reaction sheet for full instructions here).  Thankfully this tends to be very very rare. Those symptoms tend to occur within 30 minutes and warrant a call to 911 and using your epi pen.  Thankfully, this treatment tends to be safe and most patients do really well. We’re excited to get you feeling better.

Some patients will have some allergy symptoms when they take their immunotherapy- this is generally a sign that we have found the right triggers for you. With time, most patients will notice these symptoms will decrease as their immune system gets stronger. If your symptoms are bothersome, we can decrease your dose, or you can take an antihistamine like claritin, or allegra with the medication. Let us know if you’re having too many symptoms and our doctors will connect with you to create a plan.

Don’t worry! This is normal! Immunotherapy works by gradually retraining your immune system. What this means is there will be days when your allergy symptoms are non-existent, but then some times when the triggers are high and you’ll start having symptoms again. With time, most patients begin to notice that the good days far outweigh the bad days, and most patients towards the end of therapy notice very minimal if any symptoms. We measure if your immunotherapy is working by the severity of your allergy symptoms and how often you need to take additional allergy medications. Most patients will begin to notice their symptoms decrease, or that they do not need to take as many allergy medications, around 3-6 months of time. There are several steps involved in sending an at-home allergy test, getting it back to our lab, and creating your personalized allergy treatment plan.
Here’s how long each step typically takes:

1. Medical processing your order: Up to 3 business days.
2. Shipping your at-home allergy test to you: 2-4 days.
3. Taking your allergy test: Just 10 minutes!
4. Returning your allergy test to the lab: 2 business days.
5. Processing your allergy test: Up to 6 business days.

After that, you’ll get an email or text message asking you to verify your shipping and payment information. Then, you’ll receive a scheduling link to see your doctor!

For best results, complete and return your test within 14 days of receiving it. Follow the steps below to return your completed test:

1. Let the collection card dry for at least 10 minutes.
2. Once the card is dry, place it inside the specimen bag. Tip: Double-check that you’ve written your name, date of birth, and phone number on the back of your collection card.
3. Place the specimen bag back into the Wyndly box to protect it.
4. Apply the prepaid return label to the included mailer bag before placing the test box inside the mailer and sealing it.
5. Place the package in your mailbox or scan the return label to schedule a pick up to return your sample to our labs.

Once the charge goes through, your physician sends the prescription to the pharmacy. 
From there it takes about two weeks for the pharmacy to mix your custom medication and ship it to you. You should receive a tracking number once that occurs. If you did not receive a tracking number or have questions, just send us a text and we’ll get back to you. Our goal is to get your allergies better!

To ensure you do not run out of medications, our pharmacists are asked to slightly overfill your vials. Sometimes this leads to patients having extra vials of their allergy drops.

Wyndly is a concierge allergy practice. We fix your patients pollen, dust, pet, and mold allergies using a customized daily oral treatment. Instead of referring a patient to the traditional allergist where they’ll have to do painful weekly shots for 5 years, our doctors prescribe an oral treatment taken at home with convenient virtual visits.

We use the same medications as allergy shots, but administer them orally in a treatment called “sublingual Immunotherapy.” We either use FDA approved allergy extracts or FDA approved sublingual tablets and tailor each treatment based on the patient’s allergy results and history.

Dr. Manan Shah is an ENT and allergist with a practice in Denver. During the pandemic, Dr. Shah realized he could offer the same allergy care via telehealth that he was providing in the office. Two independent Cochrane review studies( 2003 and 2010) review studies have found sublingual immunotherapy to be safe and effective for environmental allergy treatment. Comparison studies between shots and allergy drops have not shown superiority of either and recommend the choice be based on patient preference. The American Academy of Head and Neck Surgery in their recent Clinical Practice Guideline noted that sublingual immunotherapy provides equivalent reduction in allergy symptoms to traditional allergy shots, with sublingual being more cost effective and convenient.

Wyndly works with patient’s 5 years and up because sublingual immunotherapy has been studied and shown to be effective in patients ages 5 and up. In fact, in children who began immunotherapy at a young age, it has been shown to decrease the risk of developing asthma later on. Below 5 years old, allergists typically do not recommend immunotherapy but rather OTC allergy medications or sprays.

Every time you send a patient to another office, it’s lost revenue and wasted time for the patient. We let providers do the blood allergy testing right in their office to ensure they are the quarterbacks of their patient’s care. Our doctors provide an included allergy test review for each test.

In addition to the cost of the test kit, most practices charge the patient an additional education and kit administration fee which we’ve seen range from $50-100. This makes it revenue positive for you, and is typically still cheaper than the copays and deductibles patients pay to get skin tested.

We use a blood allergy test kit that is clinically validated and CLIA certified.

For allergy shots, skin prick testing is required to understand skin sensitivity. For oral immunotherapy, we do not need skin test results and blood testing can be used. While QuestLab offers similar blood testing, we’ve found many patients never get their blood drawn.

To make it easier for patients, Wyndly offers an at home allergy test kit. Historically, skin testing was preferred, but with advances in technology, blood allergy testing has been shown to be equivalent to skin testing, while being more comfortable than a multi-prick allergy test. The choice of testing method should be based on patient choice and the International Consensus Statement on Allergy and Rhinology recently stated that “there is good evidence to show that [blood testing] is equivalent to skin testing” and indeed using standardized laboratory testing might actually improve accuracy.

Wyndly charges $99 a month to work with our practice. This includes the cost of all customized immunotherapy prescriptions and unlimited visits with their doctor. This is cheaper than most patients pay in copays and deductibles to get weekly allergy shots, and it saves time and discomfort.

While allergy shots typically take 3-5 years, the AAO Clinical practice guideline recommends sublingual immunotherapy for 3 years. Most research suggests both allergy shots and allergy drops have an 80-85% efficacy rate.

While allergy shots generally take 1-2 years to cause improvements, when taken consistently, sublingual immunotherapy patients generally feel decreased allergy symptoms around 4-8 months.
